Pre-Visit Checklist and Questionnaires
Before Your Appointment
Before your visit with us, please make sure to bring the following required documents with you:
- A completed pre-visit questionnaire.
- Any pertinent medical records that you may have. If these are not readily available, a "Release of Information" form will be provided at your appointment for Thomson Memory & Attention to obtain your records.
- Insurance cards.

- Select the correct link
- Download the form and save it to your computer
- Open the form you just saved to your computer
- Fill out the entire form
- Save the form
- Print the form to bring to your appointment
